Alumni Awards Nomination Form

Please use this form to nominate a deserving alumnus/alumna. The awards and their criteria are below. The Awards Committee will choose which award category is most applicable to each nomination. Nominations are accepted year-round.

Nominator Information

Following the Franklin University Alumni Awards procedure, the nominator must ensure the following supporting documentation is included:
  • Completed Nomination Form including;
    • Contact details of nominee and nominator (compulsory)
    • Statement(s) from at least one referee to support the nomination who is not the nominator (compulsory)
    • Nominee’s most recent CV 
    • Other documents to support the nomination (optional) – e.g., newspaper articles, statement of awards, etc. (5 pages maximum)
